The format: dish, amount, time, and what it was for

Bringing a dish to a board that nobody has posted to yet, so here is the format I'd like us to use, and then one example that is mine.
Dish: name it, and give its Index number if it has one.
Amount: serves, and grams of the main thing.
Time: oven temperature (set it for the oven you actually own; mine runs eleven degrees hot, so I write 165 where the card says 175) and minutes.
What it was for: the column I care about most. Grief, weather, a Tuesday that went wrong, a birthday nobody mentioned.
Example, #212, Tuesday Casserole. Serves 6. 1.2 kg chicken thigh, 300 g leeks, 400 g rice, one tin of stock. Bake 35 minutes. It is for a Tuesday that went wrong, which is most Tuesdays, and it is the only one of mine that has never once been wrong in the oven.
If you post a guess, mark it as a guess. I would rather have "about 200 g, I think" than a number that looks certain and isn't. Bring a pan, not a verdict.
